Membership Agreement and Release

In consideration for the opportunity afforded to me, the undersigned, to participate in the production tentatively entitled “Share Your Story” (the "Production") and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby grant AARP, its subsidiaries, affiliates, and their respective officers, directors, employees, contractors, licensees, successors, and assigns (collectively, “AARP”), the irrevocable and perpetual right to use, record, reproduce, distribute, transmit, display, and publish as AARP may desire, my name, likeness, photograph, voice, and comments, including, without limitation, the right to use the foregoing in connection with the Production in any manner, in whole or in part, individually or in conjunction with other materials, in any medium and for any purpose whatsoever, including, without limitation, the exhibition, advertising and exploitation of the Production.  The foregoing rights include the rights to edit my testimonial.

I understand that AARP owns the copyright and all other rights in and to the Production (including my testimonial) and may exclusively use the Production in any manner.  I also understand that I will not have any right of approval or receive compensation as a result of use of the Production.  I hereby waive any rights of privacy or publicity that I may have with respect to the Production, and release and hold harmless AARP from any and all claims and demands arising out of or in any way connected with the Production including, without limitation, any and all claims for infringement of my rights of privacy and/or publicity, defamation, and any other personal and/or property rights.  This grant and release shall inure to the benefit of the AARP, AARP Services, Inc., and their respective officers, agents, servants, and employees acting in their official capacities, and to persons, firms, or corporations affiliated with, licensed by or contracting with AARP, and their heirs, executors, administrators, successors or assigns, and to any other person lawfully using the Production or any portion thereof.

Also in exchange for the consideration set forth above, I release, hold harmless and forever discharge AARP from any and all claims, actions, causes of action, demands, rights, damages, costs, expenses and/or compensation that I may have, and/or that my estate, heirs and/or family may have against AARP arising from or in any way connected with my participation in the Production, either on or off the premises of the Production including, but not limited to, claims for personal injury and/or property damage.

I confirm that all statements that I make regarding AARP in connection with the Production will reflect my honest opinions, beliefs and/or experiences, and that any representations I may make as to the amount of any savings, discounts, or other benefits received by me as an AARP member shall be true and accurate to the best of my knowledge.

By executing this Release form by checking the box below I also represent that I have the right to agree to this Release and that I have read the foregoing and fully understand and agree to the terms and conditions set forth in this Release.  I further represent that I am over the age of twenty-one or, if I am under twenty-one years old, my parent or legal guardian has consented to the terms of this Release by checking the box on my behalf.
