The Ascension Trusted Ride Chaperone Program at Ascension Saint Agnes Hospital provides patients with roundtrip, door-to-door transportation to their medical appointments. In addition to recruiting volunteer chaperones, the grant created the Chaperone Connect Project to help the participating patients overcome social isolation and gain independence.
