Support & Train Family Caregivers Now

Family caregivers, patients, and providers all play a role in maintaining a patient’s health after hospital discharge. Yet, the way post-hospital transitions are handled can create difficulties and put a significant responsibility on the family caregiver - whether the discharge is to home, a rehab facility, assisted living, or a nursing home. 

This session is aimed at healthcare leaders, hospital and system executives, and those with influence over the patient experience. As a result of your participation you will be able to:

  • Better understand what family caregivers need related to discharge and the eventual transition from the hospital
  • Consider how age-friendly policies and practices can impact successful discharge and follow-up care
  • Drive proactive connections to community resources that build equity in the services and supports family caregivers need most
