Family caregivers, patients, and providers all play a role in maintaining a patient’s health after hospital discharge. Yet, the way post-hospital transitions are handled can create difficulties and put a significant responsibility on the family caregiver - whether the discharge is to home, a rehab facility, assisted living, or a nursing home. This session is aimed at healthcare leaders, hospital and system executives, and those with influence over the patient experience.
